Once you've completed the forms, send them along with your valid prescription to us. Download and Print Forms using your Web Browser If your having problems downloading our forms in Adobe Acrobat format, click the above link to download the forms in your web browser. After you've printed and completed our forms, you can Fax (recommended) or mail them to us.

We'll MAIL you the forms If you don't have access to a printer Call 1-800-PILLS-81 (1-800-745-5781) or complete our online mail request form and we'll send you the required forms in the mail. You can then either Fax (recommended) or mail the completed forms back to us.
